Summer Academic Program:  Non-LT Student Information Form

Participants of the 2010 Summer Academic Program that do not attend Lyons Township High School must complete a non-LT information form and submit it prior to the start of the Summer Academic Program on June 14, 2010.  Completed forms can be submitted in two ways:

  • Completed online, printed out and mailed to Lyons Township High School
  • Completed online, printed out and submitted electronically to Lyons Township High School
    • Please note:  If you use a web-based email application (hotmail, gmail, yahoo, etc.), you will need to save the file to your computer and send it through email as an attachment. Please email completed forms to summerforms@lths.net  .

Summer Activity Program:  Summer Emergency Form

Participants of the 2010 Lyons Township High School Summer Activity Program must complete an emergency form and submit it prior to May 31, 2010.  Completed forms can be submitted in two ways:

  • Completed online, printed out and mailed to Lyons Township High School
  • Completed online, printed out and submitted electronically to Lyons Township High School
