Fall Sports :::

ATHLETIC PROGRAMS - FALL SEASON - 2007

All students wishing to participate in athletics must have a current physical and complete an Athletic/Insurance Student Activities form. This form must be presented to the coach or sponsor of the activity on the first day of practice. The Athletic Department requires each participant to complete a new form for each sport season. This insures the safety of the athlete, and also updates the athletic eligibility files. The form will be sent home in early June as part of the all school mailing or you can print forms by going to Lyons Township High School ’s Web Site at www.lths.net – click on athletics, then click forms. Physical forms can also be found here.
