To request a transcript/immunization records, please complete the below form and return it to my attention with $1.00 per transcript/immunization record requested to:

You may also return it in person to the Records Office at North Campus which is located across from the Asst. Principal’s Office and Career Center (next to the counselors). My hours are 7:30 to 3:30 but please call before you come to make sure I’m in the office and not at lunch or an appt. You may fax the release form but note that you are still responsible for the fee which can be mailed or dropped off by a third party.
 
If you are requesting more than one transcript/immunization record, just write them all on this one form. Use the back or additional paper if you need to.
 
If someone other than yourself is picking up the transcript/immunization record, separate written permission must be given for that person to do so.
