Submit your completed sport physical to healthservices@lths.net.

Can't recall when your sports physical expires? Check in Infinite Campus (instructions)

All other forms need to be submitted electronically as you register for each specific sport.

You do not need to print or turn in any of these forms. They are a part of the online registration process and the Athletic Office will get the forms when you submit your registration.
