Freedom of Information Act Request for Documents

FOIA is the Illinois Freedom of Information Act. Under the Illinois Freedom of Information Act 5 ILCS 140/1 et. seq, records in possession of public agencies may be accessed by the public upon written request.
 

The following are links to documents available on the District website which you may download at this time (no FOIA request required):

HOW CAN I SUBMIT A FOIA REQUEST?

LTHS has no specific FOIA request form. Requests should state that information is being sought under the Freedom of Information Act. Provide a description of the public records requested, being as specific as possible. While responses may be provided via e-mail, you must provide a mailing address.

All requests for records should be in writing and addressed to FOIA Officer Mary Lin Muscolino. Individuals making a request are to identify when the request is for a commercial purpose or when requesting a fee waiver.

What happens after I submit a request?

The Illinois Freedom of Information Act requires agencies to respond within five working days of receipt of a request. Please note any requests that are submitted after 3:05 pm will not be reviewed until the following business day. A five-day extension is allowed with written notification to the requester.

If the requested records are 50 pages, or less, in length, the pages will be copied and provided to the requestor. If the records exceed 50 pages, the requester will be informed of the duplication cost.

Fees must be collected by the District before copies will be made or sent to the requestor. 

Is there a process to appeal the FOIA officer's decisions?

In the event that part or all of a request is denied, requestors have the right to have the denial reviewed by the Public Access Counselor's Office of the Illinois Attorney General or by filing suit in circuit court.
