HOW CAN I SUBMIT A FOIA REQUEST?
LTHS has no specific FOIA request form. Requests should state that information is being sought under the Freedom of Information Act. Provide a description of the public records requested, being as specific as possible. While responses may be provided via e-mail, you must provide a mailing address.
All requests for records should be in writing and addressed to FOIA Officer Mary Lin Muscolino. Individuals making a request are to identify when the request is for a commercial purpose or when requesting a fee waiver.

What happens after I submit a request?
If the requested records are 50 pages, or less, in length, the pages will be copied and provided to the requestor. If the records exceed 50 pages, the requester will be informed of the duplication cost.

Fees must be collected by the District before copies will be made or sent to the requestor.
Is there a process to appeal the FOIA officer's decisions?
Additional Information
“Junk mail” (unsolicited commercial email or communications and not responded to by an employee of the public body) is not a ”public record,” and need not be disclosed.
Electronic FOIA requests must include the request in the body of the email. The District is not required to click on a hyperlink or open an attachment to view the request. If the District receives a request necessitates the clicking on a hyperlink or attachment to view the request, the District must notify the requester with in 5 business days that the request must appear within the body of the email.
If the District has a reasonable belief that the requester is not a person, the District may send a notice within 5 business days requiring the request to verify orally or in writing that the requester is a person. Failure of the requester to respond within 30 calendar days is grounds for denial of the request. The District may not require the requester to submit personal or private information or identifying information to verify that they are a requester.
