Annual Statewide Illinois 5Essentials Survey

We would like your feedback through the annual statewide Illinois 5Essentials Survey. This online survey will help school and district staff understand parent/guardian perceptions regarding communication, school/family partnerships, and school culture and climate.
Individual participation and responses are confidential. If 20% or more of parents/guardians participate, aggregate results will be available to school staff for review and published online in the fall of 2025 with the annual Illinois Report Card. The results of this survey will be used to understand areas of strength, opportunities for growth, and to support the district’s strategic plan.
The survey window closes on Friday, March 28, 2025.
