Physical, Dental and Vision Exams - does my student need one?

Incoming freshman require documentation of a health examination with proof of immunizations on the IL Certificate of Child Health Examination form within one year prior to entering ninth grade. This document must be on file in the school health office prior the first day of school. If it is not on file by the first day of school, the student will be excluded from school. This exclusion shall begin immediately and shall remain in effect until the student is in compliance. In addition, documentation of a dental examination by a licensed dentist on the IL Dental Examination form within 18 months prior to entering ninth grade is due by no later than May 15 of the student’s freshman year.

Transfer students from within Illinois have no grace period and are expected to comply with Illinois School Code upon enrollment.

Transfer students from out of state or out of the country require documentation of: a health examination with proof of immunizations on the IL Certificate of Child Health Examination form, a dental examination by a licensed dentist on the IL Dental Examination dental exam and documentation of a vision exam by a licensed optometrist or ophthalmologist within 30 days from enrollment.
