Incoming Freshman Health Requirements

Incoming freshmen require documentation of a health examination with proof of immunizations on the IL Certificate of Child Health Examination form within one year prior to entering ninth grade.This document must be on file in the school health office prior to the first day of school. If it is not on file by the first day of school, the student will be excluded from school. This exclusion shall begin immediately and shall remain in effect until the student is in compliance.

All incoming freshman are required to submit:

1. Health examination with proof of immunizations prior to the first day of school. This health examination must be completed by a MD, DO, APN or PA on or after 8/19/23 and documented on the Illinois Certificate of Child Health Examination. 
2. Dental examination no later than May 15, 2025. This dental examination must be completed by a licensed dentist on or after 11/15/23 and documented on the IDPH Dental Examination Form.

Students with health needs during the school day:

• Any medication required during the school day, including over the counter medication, requires a Medication Authorization Form and must be stored in the health office. The only exceptions are epinephrine auto-injectors, quick relief asthma inhalers, and diabetic supplies/insulin.
• Students with asthma, diabetes, epilepsy, and life threatening allergies require additional medical documentation. For a specific form, please visit Health Forms.

Electronic submission of health documents is preferred. Electronic documents must be submitted as PDF or JPEG. You may upload health documents during online registration or email documents to HealthServices@lths.net: Please include student name and grade level in subject line when submitting forms, ie Kathy Weber Grade 9
