PE Medical Restrictions Information

Lyons Township High School believes in the importance of regular physical activity with various studies having shown the positive impact of physical activity on students' physical, mental, and academic wellbeing.Due to this, we strive to keep all students in physical education classes whenever possible.

If a student requires restrictions from certain physical activities due to a short-term illness or injury OR a longer-term diagnosis that could impact their successful participation in Physical Education courses, please have your physician complete the  Physical Education Medical Limitation Form. 

Once completed, please submit the form to our Health Services Office (JPEG or PDF document to: healthservices@lths.net) so our staff can evaluate the restrictions and work with our Physical Welfare Division to provide the appropriate accommodations or waiver if necessary.  Please read the form carefully so you understand the process and how these restrictions can impact athletics and activities.  The form must be completed and signed by the physician treating the condition.
